Mental health together

A new NHS service called Mental Health Together will soon be available to patients in East Kent in the latest phase of its roll-out across Kent and Medway.

Following a first phase roll-out in Medway and Swale, Mental Health Together will be available to support people in Thanet before continuing to Ashford and Canterbury, and then other coastal areas.

Previously, some services have been hard-to-reach and operate in a siloed way. Mental Health Together has been designed to bring these services together to support people with mental health difficulties so they can live well in their communities.

A range of new interventions will be available to support someone’s mental health needs, including lived experience-led workshops, new trauma-informed psychological programmes and improved physical health care.

Referrals to the service will be made to a Mental Health Together team in each local area, who will then collaboratively create a multi-agency support plan. The service is designed so all organisations and agencies, including those in the voluntary sector, can work much closer together to provide someone’s care in a joined-up way. It is a holistic approach, which means mental health, physical health, and social parts of life - including employment or housing - are supported within one care plan. This ensures that people get the right care, without having to navigate service boundaries and without repeatedly telling their story.

The service will also provide help to families and carers who are supporting someone with serious mental health needs.
